 Immigration Exams

 Form I-693 is used to report the results of a medical exam for public health admissibility to the United States. Only a designated civil surgeon approved by the USCIS (US Citizenship and Immigration Services) may conduct the immigration medical exam. Generally, all applicants filing for adjustment of status to that of a lawful permanent resident must submit Form I-693 to the USCIS.

Schedule your exam  We will do our best to accommodate your schedule. Our clinic is open 7:30 a.m. to 4 p.m., Monday, Wednesday, Thursday, and Friday. The cost for adults is $545 and for children (0–14 years) $445. Included in this price are required lab tests  such as RPR Syphilis, Gonorrhea, and Quantiferon TB gold. We accept credit, debit, or cash at the time the appointment is scheduled. Should further x-rays, immunizations, or lab work be needed or requested, additional payment may be paid at time of service. Please schedule your appointment time carefully to avoid a $100 missed appointment fee. We look forward to guiding you through the examination process with confidence.

Immunizations: Please bring your written and dated (month/day/year) documentation of your immunization history to your first appointment. If the information is not available you will have to get additional, perhaps unnecessary, shots. 

Tuberculosis: If you have been treated for TB you must bring a letter or record of proof of treatment details and duration. Contact the health department who provided this treatment, if you do not have this documentation. They may fax our office at 360-757-3698 in advance of your first appointment.

  • Documentation of immunization
  • Valid government issued photo ID for adults
  • For child under 14, ID containing name, birthplace, birthdate, and names of parents with translation
  • English speaking adult translator if translation is needed
  • TB treatment record if applicable
