On July 22, 2011, a new state law changed the rules for vaccination exemption. Children must submit a Certificate of Immunization Status before attending a licensed child care or school. The law has changed for those who want to exempt their child from immunization requirements. Parents or guardians must be counseled by a licensed health care provider about the benefits and risks of immunization. The provider will sign a Certificate of Exemption form which the parent or guardian may submit to the child care or school. (Exception: A health care provider does not need to sign the form for parents or guardians who show membership in a church or religious body that does not allow medical care from a health care provider.)
